Game is Diablo III: Reaper of Souls on PC naturally. Apparently Youtube cannot handle 4K at 60fps yet. 720p and 1080p shows 60fps, but it kind of bugs out when selecting 2160p. UPDATE: Now Youtube has removed 4k quaity option for this video altogether.
